tracking data, analyzing results and preparing reports; supervisory

As per contractual agreements, provide agreed upon industrial EHS
evaluations and recommend appropriate actions; develop and implement
appropriate action plans based upon physical plant, equipment and safety
policy reviews, Job Safety Analysis and equipment inspections, Proactive
Risk Assessments, Performance Improvement Plans, Failure Mode Effects
Analysis and other diagnostic tools; Develop and facilitate employee safety
training modules and manage the EHS Programs.
9/2005-8/2010: United Hospital System, Inc., Kenosha, Wisconsin
Patient Safety Officer
Managed the United Hospital System, Inc. (UHSI) Safety Department, as well
as The Joint Commission (TJC) Environment of Care, Life Safety and
Emergency Preparedness Management Plans; and participated in two TJC
accreditation surveys by answering questions concerning those plans and by
providing the appropriate documentation and site tours in 2 medical center
campuses and 5 clinical practice centers that make up UHSI. Also completed
the NIMS-related IS-100HC, 200HC, 700A and 800B levels training courses and
was assigned as Safety Officer and Liaison Officer in the hospital incident
command system.
Reviewed building plans for an expansion project of one of the System's
medical center campuses and was responsible for Interim Life Safety
Measures (ILSM) and Infection Control Risk Assessment planning and follow-
through, as well for as the conduct of daily Safety Officer Construction
Site Inspections and air particle sampling, quarterly ILSM fire drills and
the inventorying and documenting all existing and new life safety systems,
including fire alarm and fire protection devices installed at both UHSI
medical center campuses and the development and installation of ILSM Fire
Exit Plans in health care areas adjacent to or affected by new construction
activities.
Conducted Proactive Risk Assessments and Performance Improvement Plans and
assured that appropriate recommendations of those reports were implemented.
Assigned Environment of Care Committee (EOC) staff members and facilitated
in the development of Failure Modes and Effects Analysis reports (FMEAs) to
analyze and prevent disasters.
Monitored/maintained the UHS electronic Statement of Conditions (eSOC} on
the JC Website for a critical self-assessment of current levels of
compliance and to describe how to resolve any Life Safety Code (LSC)
deficiencies.
Provided the EOC Committee with Safety Officer Reports during the monthly
EOC Committee Meetings; EOC Dashboard, Fire Drill, Life Safety and Safety
Management Subcommittee and Safety Reported Hazard Reports on a quarterly
basis; Environmental Tour/Risk Assessment Reports on a semi-annual basis
and the 7 JC Management Plan Reviews on an annual basis.

recommendations as appropriate for identified deficiencies.

Drafted and updated written corporate safety policy and ESH program
procedures, in accordance with EPA, OSHA, NFPA and WDNR standards and
codes.
Assisted Human Resources by maintaining an open dialogue with WC Insurance
Provider Safety Specialists and maintaining the OSHA 300/301 Logs; and by
assuring that injured workers returning to their jobs with injury
restrictions were monitored under Worker's Compensation guidelines.
As Chair of the Safety Committee, I facilitated committee members in
reviewing reported employee safety incidents and injuries, and in
developing corporate safety fairs and events to keep safety as a priority
in the minds of CityForest employees.
Managed preparedness of the corporation's First Response and Fire Safety
Teams; and developed and conducted outside training guidelines, materials
and pre/post tests for the monthly team member safety training meetings.
Developed and presented EHS program status reports at Management, Team
Leader and Process Area Leader Meeting; and developed and conducted the new
