specific medical documents, i.e., sleep studies, EKG, EEG, PFT, and other clinical notes to be
scanned from the military and civilian heath care facilities. Knowledge of HIPPA and other
Privacy Health Information rules and regulations IAW Army Regulation 40-66. Receives
telephone calls from Military Treatment Facilities, civilian facilities, and patient relating to
eligibility requirements, and location of medical records. Releases medical records upon request
to military personnel, family members, and military and civilian facilities according to
regulations. Retrieves medical records and loose medical documents from wards, and
administrative areas for appropriate distribution. Enters loose medical documents in individual
patient health records according to established guidelines and procedures. Sort researched reports
for mail outs to various MTFs- CONUS/OCONUS. Remain proficient on all applicable data
systems (AHLTA, CHCS, ESSENTRIS). Searches for missing medical records and assist in

quality control to ensure inclusion and completeness of notes, nurse notes, physician orders and
arrangement of records contents in prescribed sequence. Utilizes the ICD-9 manual to determine
diagnostic and operative terminology used is consistent with medical nomenclature. Consults
with medical staff to rectify apparent inconsistencies and discrepancies. Medical records are
assembled and forwarded to coding. Participates in the referral of select cases to appropriate
committee as identified in the review and analysis. Read lab test, graphic records, provider's
