Summary: Experienced hospice and palliative care nurse seeking nurse
practitioner position where my skills can be used to provide supportive
care to chronically ill and dying patients. Recent adult geriatric NP
graduate with clinical rotations in a fast-paced oncology/hematology
practice and hospital-based palliative program. Both rotations included
experience in providing hospital consults under preceptor supervision.
Spent an additional eight months working in busy family practice under
guidance of FNP and MD.
Nursing Experience
Hospice and Palliative Care: Forbes Hospice
(May 2007 to present) Certified Hospice and Palliative Care Nurse (CHPN).
Assess patients for signs and symptoms of terminal disease and provide
appropriate palliative care for patients who choose to manage their
illnesses at home. Positions held: Weekend On-Call, Admissions, and Case
Manager. Also manage virtual hospice patients in hospitals, facilities and
personal care homes. Collaborated with palliative care program medical
director and associated nurse practitioners, attending MDs, case/care
managers, pharmacists, and social workers. Spent last semester of NP
clinical rotation with hospital-based palliative care program for large
health care system.
Rehab: Forbes Regional Hospital
(December 2005 to May 2007) Assessed patients, passed medications, charted
electronically and on paper, provided full patient care and worked closely
with physical therapists, occupational therapists, speech therapists and
social workers. Patients were recovering from surgeries, neurological
events and unspecified debilitation. Worked full time and then flex in this
unit.
Psychiatric RN: Braddock Hospital and Mon Yough Community Health
(May 2006-May 2007) Staff RN on busy dual -diagnosis unit at Braddock
Hospital. Community Mental Health Nurse for non-profit agency working with
patients with severe mental illness who were attempting to transition from
facilities to independent living.
Oncology/Hematology: Nurse Extern and NP student at Forbes Regional
Hospital
(May 2005 to September 2005 & September 2012 to November 2012) As pre-RN
nurse extern (2005): Provided patient care, passed medications, and charted
under the supervision of preceptor nurse. As NP student (2012): Assessed
patients in hematology/oncology practice, including training experience in
providing hospital consults.
