MedAdventures is seeking an experienced Home Health Physical Therapist to join our allied health team and provide high-quality care to patients across the Meyersdale area. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact in the lives of patients receiving care at home.

Schedule: Flexible – full-time or part-time options

Mileage: Reimbursed

Caseload: Adult and geriatric population

(Agency coverage allowed within 1 hour of the Meyersdale office and when wounds are not complex or requiring IV support.)

Responsibilities

Evaluate, treat, and manage home-based physical therapy patients

Develop individualized care plans

Collaborate with the clinical team and communicate progress

Document timely and accurately in accordance with home health standards

Provide patient and caregiver education to support safe, effective care

Qualifications

Active Pennsylvania Physical Therapist license

Minimum 1 year of home health experience preferred

Strong clinical judgement and ability to work independently

Reliable transportation and valid driver’s license
