Job Summary

As a Substance Abuse Clinician, you will collaborate with interdisciplinary teams to deliver evidence-based clinical services. You will uphold ethical principles and professional standards to ensure the best outcomes for our patients.

Key Responsibilities

Conduct comprehensive bio-psychosocial assessments and create tailored care plans.

Work with the interdisciplinary team to evaluate care levels, identify risks, implement care plans, and facilitate referrals to enhance clinical and financial outcomes.

Utilize effective therapeutic strategies to support individualized care plans.

Monitor patient progress and adjust treatment plans to align with evolving needs.

Provide education to patients and families regarding disease prevention, treatment adherence, and health maintenance.

Deliver direct patient care services, including case management, crisis intervention, counseling, and discharge planning.

Refer patients to necessary resources that enhance their care journey.

Ensure timely and thorough patient documentation in compliance with regulatory standards.

Provide clinical social work interventions across diverse patient populations independently.

Engage in team meetings, clinical supervision, and staff education activities.

Supervise social work students in their field placements.

Contribute to program development to meet patient and unit needs.

Collaborate with team members to enhance overall patient experiences.

Utilize resources efficiently to ensure cost-effective care delivery.

Qualifications

Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or a related human service field.

Prior experience in a healthcare setting.

Valid LCSW and LADC licenses required.

Valid Driver's License.

Proficient in computer use.

Ability to assess issues and make timely decisions.

Strong communication skills and relationship-building abilities.

Demonstrated competencies to meet the needs of various patient age groups.

Commitment to personal and patient safety.

Proactive in professional development and staying updated with relevant certifications and training.

Awareness of health-related social needs and cultural competency, addressing inequalities in healthcare.

Ability to navigate cultural differences and integrate this understanding into practice.
