The Behavioral Health Counselor provides a full range of integrated casework, including psychosocial assessments, person-centered treatment planning with a harm reduction orientation, evidence-based clinical interventions, advocacy, and referrals to individuals who present with a variety of mental health concerns, co-occurring substance use disorders, chronic health conditions, and concerns around gender identity and sexual orientation. They must be proficient in trauma-informed assessments and treatment.
As part of the essential functions of this role, the Behavioral Health Counselor:
Conducts initial psychosocial assessments either on-site and provides initial treatment recommendations, and collaboratively with the patient develops a personalized treatment plan with emphasis on evidence-based practice within a harm reduction orientation.
Provides resources and referrals as indicated to facilitate achievement of a patient’s treatment goals.
Provides psychotherapy and crisis intervention to individuals and families who are dealing with a variety of issues, including but not limited to mental health, including trauma, co-occurring substance use disorders, chronic medical conditions including HIV, HCV and issues related to sexual orientation and gender identity
Adheres to program expectations and requirements surrounding productivity, documentation timelines and standards, and overall quality of patient care
Works within an integrated treatment model coordinating treatment with both internal and external providers as indicated
Will serve as a role model for positive, productive, and cooperative relationships with staff and clients
Adheres to Office of Mental Health regulations and Evergreen Health policy and procedures, including regulatory timeframes on clinical documentation using an electronic medical record
Maintains confidentiality of all treatment records
Qualified Candidate will have a Master’s degree in social work (or similar), two (2) years of relevant experience, with NYS license required- LMSW or LMHC, LCSW preferred; candidate must have strong communication and organizational skills; candidate must have knowledge of Diagnostic and Statistical Manual of Mental Disorders; experience utilizing electronic medical records software (Medent) strongly preferred; sensitivity to and experience with underserved populations is essential.

Education and Experience: Master’s degree in social work (or similar), NYS license required- LMSW or LMHC at minimum, LCSW preferred, and two (2) years of relevant experience required, with LMSW/LCSW preferred
