Salary & Benefits:

Salary Range: $105,000 - $135,000 (Experienced providers with bonuses can earn $150,000 - $160,000)

Annual Clinical Performance Bonus:

Less than 5 years of experience: $15,000

5+ years of experience: $25,000

Reimbursement for Mileage and Toll Fees

Role: Transitional Health (Home Health)

Focus: Post-discharge visits, including:

Medication reconciliation

Social service follow-ups

General follow-up care and care coordination

Caseload: 6-7 patients per day

Support: Medical Director available for guidance

Required Qualifications:

Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program graduate

National certification in at least one of the following specialties:

Family Nurse Practitioner

Adult-Gerontology Primary Care Nurse Practitioner

Adult Nurse Practitioner

Gerontological Nurse Practitioner

Active, non-probationary state Nurse Practitioner license

Active DEA license

US work authorization
