Job Description:

This position provides home health care services to patients in their homes across the assigned travel area. The RN will manage patient care using Homecare Homebase (HCHB) and OASIS documentation, ensuring high-quality, patient-centered care. Responsibilities include clinical assessment, care planning, and coordination with interdisciplinary teams.

Qualifications:

Current Registered Nurse license in the state of Georgia

1–2 years clinical RN experience required; 2+ years home health experience preferred

HCHB, OASIS, and home health experience required

CPR certification required; CHPN certification preferred

Valid driver’s license, reliable transportation, and insurance required

Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program; BSN preferred
