Overview:
The School-Based Case Manager provides mental health support and care coordination to students within a school setting, in partnership with a community mental health center. They will utilize evidence base programs to provide individual and group social and emotional learning support and education. This role involves identifying students with emotional, behavioral, or mental health needs and connecting them with appropriate services and resources. The case manager collaborates closely with school staff, families, and community providers to ensure continuity of care, support student success, and promote overall well-being. Responsibilities include conducting assessments, developing individualized service plans, linking students to therapy or other services, and maintaining accurate documentation in accordance with agency and funding guidelines. The ideal candidate will be compassionate, organized, and able to work independently within a school environment, with a strong understanding of youth mental health and trauma-informed care.

Qualifications:
Psychologist with a master's or doctoral degree from an accredited program, OR Licensed Social Worker, OR Licensed Registered Nurse, OR a master's or bachelor's degree granted by an accredited college or university in a human services-related field, including disciplines approved by the West Virginia Board of Social Work.
Valid driver's license and satisfactory driving record Job Type: Full-time
