Position Overview
Entry level position requiring working knowledge of resident care. Under general supervision, provides basic care of residents with Intellectual Disabilities and/or Developmental Disabilities; participates in the development and modification of Individualized Service Plan for the residents; teaches residents independent living skills, as defined in their service plan.
Summary of Job Duties
Provides individualized assistance to residents on a daily basis, according to the Individual Service Plan. Some areas of focus in the Individual Service Plans are Self-Help (i.e., toileting, bathing, teeth brushing, face/hand washing, grooming, dressing and dining behavior); Communication (i.e., verbal, vocal, gestural, manual and augmentative expressive skills; and understanding directional requests for receptive skills); Socialization (i.e., appropriate interaction with others, participatory skills, developing friendships and community behaviors); Leisure/Recreation (i.e., independence with age-appropriate instructional/leisure materials, developing interest in sensorimotor activities, participating in community events and cooperative play or recreation activities). These skills are non-exclusive and based on the individual needs as defined by the Individual Service Plan. Responsible for resident accountability by recording daily progress; interacts in a positive manner with residents at all times.
Demonstrates empathy, compassion, patience, and foster a caring bedside manner; have the ability to discern a patient's non-verbal cues, as well as physical and mental health changes; communicate effectively by understanding how, when and where communication may be appropriate in certain situations; remain flexible in the face of changing duties and /or requirements; must be able to adapt to changing personalities and negative situations. Knowledge of and ability to carry out emergency procedures.
Ensures the safety and wellbeing of residents; supervises and records food and liquid intake, records elimination, and provides repositioning every two (2) hours or more as needed to prevent deterioration. Monitors vital signs; observes, notes and reports any significant changes in resident behavior or condition requiring medical attention. Performs basic nursing functions; maintains accurate medical documentation as required. Monitors the safety of residents in room or bed regardless of direct involvement in service plan; may assist residents during unscheduled occurrences in bathrooms, dining rooms, halls, or indoor and outdoor environments.
Assists in the development of the Individual Service Plan. Attends Individual Service Plan meetings, staff meetings, scheduled trainings, and staff in-services. Maintains personal effects of residents; assists in cleaning program areas, wheelchairs and other adaptive equipment of residents; carries out daily living activities and may be required to escort residents to and from scheduled activities and appointments (e.g., community outings, doctor appointments).
Maintains a clean and orderly living area; assists with serving meals and notifies management of equipment needs and repairs. Completes minor household repairs; maintains accurate and up-to-date records of resident progress (e.g., menu planning, daily log reports, etc.). Establish appropriate relationships with resident families and guardians.

Minimum Qualifications and Requirements
Habilitation Provider - Entry
High School diploma or equivalent.
Habilitation Provider -
High School diploma or equivalent and six (6) months experience in habilitation field working with handicapped or severe to profound developmentally disabled individuals, or six (6) months resident care nurse aide experience in a long-term care facility or six (6) months home health care experience with Certified Nursing Assistant (CNA) or State Tested Nurse Aide (STNA) certification - OR - alternative, equivalent evidence of the Minimum Class Requirements.
Supplemental Information
1st Shift (6:00 am - 2:00 pm) or 2nd Shift (2:00 pm to 10:00 pm)
Driving Requirement:
May require current valid United States driver's license with an acceptable driving record and proof of insurance.
Physical Requirement:
Must be able to lift up to 50 lbs. and demonstrate full range of motion for bending, stooping and lifting to provide patient assistance.
Training & Development Required:
New Employee Orientation training in the care of residents with developmental disabilities, CPR and First Aid Certification must be successfully completed during initial probationary period and maintained during employment; may require certification in medication administration.
All full-time, part-time, temporary, and intermittent employees of Montgomery County working at Stillwater Center are required to be fully vaccinated against COVID-19. New hires must provide a copy of their vaccination card to the Stillwater Center HR Manager prior to their employment start date or receive an approved exemption from the Director of Human Resources.

Seasonal Employees: Benefits include Sick Leave accrued at 4.6 hours for every eighty 80 hours in active
pay status. Seasonal employees scheduled to work 40 hours during their period of employment will accrue vacation in the same manner as regular full-time employees and can use their vacation after completion of twenty-six (26) bi-weekly pay periods in active pay status. Part-time Seasonal employees (scheduled to work less than 40 hours) do not accrue vacation.
Intermittent and Temporary Employees: Long-Term Sick Leave accrual is based on hours worked.
