Under the guidance of the Director of Care Management, the Associate Community Health Worker (CHW) plays a vital role in helping patients access and navigate community services. This position involves outreach, providing information and support to community members, and assisting in data collection to identify health needs and promote improvements, especially focusing on high-risk populations.

Essential Duties and Responsibilities

Build and maintain strong relationships with community partners, patients, and their families while supporting healthy behaviors.

Assist families, groups, and communities in accessing social and health-related services.

Assess client needs and refer them to appropriate community resources and agencies, developing processes to evaluate Social Determinants of Health in various settings.

Participate in community meetings and events such as health fairs to understand local issues and share resource information.

Advocate for individual and community health needs.

Serve as a liaison between healthcare providers, community organizations, and the populations served.

Develop referral and follow-up processes for identified patient needs.

Distribute educational materials tailored to the target community.

Perform other duties as needed to support organizational goals.

Education

An associate degree in public health, social work, psychology, sociology, or related fields such as communications, education, or business is required.

Experience

Experience with social services or public health systems is preferred.

Certification and Licenses

Basic Life Support (BLS) certification must be obtained within the first 90 days of employment (training provided). Certification as a Community Health Worker must be earned within the first six months.

Mental and Physical Requirements

This role requires considerable mental focus for extended periods with frequent interruptions. Light lifting up to 15 lbs is necessary. Activities include standing, sitting, walking, reading, and speaking.

Working Conditions

Typical office environment with some noise and distractions. Possible eye strain from computer use. Attendance at public meetings is required. There may be exposure to infectious or communicable diseases while interacting with patients in various settings.
