The Primary Care Provider is responsible for delivering comprehensive primary healthcare services to the patients of the Sokaogon Chippewa Community Health Clinic. This role involves assessing, diagnosing, and treating a wide range of health issues while promoting wellness and preventive care. The provider will work collaboratively with a multidisciplinary team to ensure culturally competent and accessible healthcare for all patients.

Duties:

Duties and Responsibilities: • Conduct thorough assessments and evaluations of patients' health conditions. • Diagnose and manage acute and chronic illnesses, providing evidence-based treatment plans. • Offer preventive care services, including health screenings, vaccinations, and health education. • Understand and respect the cultural values and traditions of the tribal community. • Engage with patients in a manner that fosters trust and communication, ensuring that care is tailored to individual needs. • Work closely with other healthcare professionals, including nurses, mental health providers, and specialists, to coordinate comprehensive patient care. • Participate in interdisciplinary team meetings to discuss patient cases and improve service delivery. • Actively engage with the community to promote health awareness and education. • Conduct outreach programs and workshops to address specific health issues prevalent in the community. • Maintain accurate and confidential patient records in compliance with legal and ethical standards. • Ensure compliance with all relevant health regulations, clinic policies, and quality assurance protocols. • Stay current with medical best practices, guidelines, and advances in primary care. • Participate in training and professional development opportunities to enhance skills and knowledge. • Must have a working knowledge and be able to adhere to all tribal ordinances, policies and procedures, EPA permitting process, THPO permitting process and any other regulations that are applicable to the grant projects. • Complete other duties as assigned by Medical Director.

Qualifications:

Qualifications (Education, Experience, Knowledge, Skills, Abilities): • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution. • Current state medical license and board certification in family medicine, internal medicine, or a related specialty. • Experience in primary care, preferably in a community or tribal health setting. • Strong communication and interpersonal skills, with a commitment to culturally competent care. • Ability to work collaboratively in a team-oriented environment.
