Overall Function:

Provide support to clients/residents with mental health needs who have been incarcerated at the county, state, or federal level to integrate into transitional housing to offer a choice between custody and independence.

Essential Functions:

Provides hands-on instruction to residents to implement Individual crisis plans and assist with crisis resolution.

Provide support by aiding clients to live as independently as possible and reach obtainable goals to maintain their independence.

Aids and support with the resident's Activities of Daily Living and Instrumental Activities of Daily Living as indicated in the resident's assessment and support plan.

On a day-to-day basis, support with meal preparation, household chores, medication management, budgeting and finances, accessing community facilities/activities, shopping, socialization, doctor's appointments, and providing emotional support.

Assist the client with skill development and job placement.

Monitor residents' progress and complete documentation, identifying problem areas that may need to be addressed.

Participates in regular staff training to include specialized training, to develop basic knowledge and skills to assist residents in crisis resolution, and support job performance.

Responds appropriately to emergency situations.

Maintains proper and detailed documentation of all assistance and support provided to each resident.

Other duties as required

Adheres to all policies, laws, regulations, and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures

High School Diploma, GED, or an equivalent combination of skills, experience, and education may be considered at the discretion of the Agency.

Ability to receive, understand, and communicate information with effectiveness and follow instructions.

Must have a valid Pennsylvania Driver's License and a safe driving record.

Must have the ability to utilize electronic medical records.

Reliable, organized, and committed to the health and safety of the clients.

Displays a positive and compassionate attitude and manages multiple tasks at one time.

Must successfully obtain Criminal History Clearance from the PA State Police and Child Abuse Clearance from the PA Department of Human Services, and FBI Clearance.

Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.
