Basic Purpose of the Job

The social worker acts as a patient advocate to the hospital clients. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes. Determines medical and social services eligibility on new admissions. Interviews clients to assess client's social situation; identifies psycho-social problems that would interfere with treatment plan, provides supportive counseling to clients, families and staff members; assists them in understanding client's physical and/or mental disability; encourages and motivates patient and family to actively participate and cooperate in treatment plan. Conducts home visits for follow-ups or home assessments to ensure client's treatment plan will be successful. Coordinates with other agencies and organizations in the provision of services for clients or family. Works collaboratively with interdisciplinary staff internal and external organizations. Participates in quality improvements and evaluation processes related to the management of patient care. The social worker is on-site and available seven days a week and during holidays, therefore, is required to work an on-call rotation schedule.

Minimum Education

Bachelor's degree in Social Work

Master's degree in Social Work preferred.

Graduate from an accredited school preferred.

Required Licenses / Certifications

Current Healthcare Provider BLS Certification

Required Skills, Knowledge and Abilities

Knowledge of social work theories, principles and practices.

Ability to understand the dynamics of individual and group behavior.

Ability to apply and interpret rules and regulations, and procedures pertaining to social work.

Ability to develop and implement individual treatment plans with flexibility and independence.

Ability to utilize relevant personality theory, casework method, consultation in social work practice.

Ability to interpret programs to other staff, discipline, and community agencies.

Ability to analyze information and to make work decisions based on this information.

Ability to work effectively with the public and staff members.

Ability to communicate effectively, orally and in writing.

Ability to maintain records and prepare reports.

Skill in the safe operation of a motor vehicle.

Knowledge of post-acute care community resources.

Demonstrate flexibility via an ability to adapt to changing priorities and regulations.
