POSITION SUMMARY

The Triage and Follow-Up Specialist functions as an integral part of the Mobile Crisis Program and focused on assisting children and youth who are in crisis. Provides support to callers with focus on coordinating and triaging clinical staff to the appropriate response.

EDUCATION AND EXPERIENCE/QUALIFICATIONS

High school diploma plus at least 2 years of experience in human services field preferably in crisis intervention is required. Experience can be substituted with education on a year for year basis, if the degree is in a related field. Knowledge of substance abuse and/or mental health services, crisis management and data entry are preferred.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Answer Crisis Calls to coordinate and triage calls for clinical staff.

Follow up with callers to schedule appointments and coordinate care.

Maintains professionalism towards all providers and other staff members; contributes to a positive team culture.

Completes documentation for all calls in an accurate, timely and thorough manner.

Ensures completion of pending items related to calls in a timely manner. When unable to complete a task, ensures that other staff members are aware of the task requiring completion.

Maintains awareness of high-risk clients and implements appropriate interventions as defined in alerts and crisis plans.

May assist in orienting new staff.

Keeps abreast of program information and implements identified changes.

Attends all trainings and staff meetings as required. Completes all online trainings in a timely manner and as required.

Maintains confidentiality in a professional manner of all information received, discussed, and recorded.

Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
