What You'll Do:
Travel daily to assigned MN service areas to visit clients in their private homes
Assess communication and swallowing disorders, then develop and implement personalized plans focusing on dysphagia, speech intelligibility, and cognitive-communication needs
Provide interventions based on physician orders and educate caregivers in safe feeding techniques and communication strategies aligned with the care plan
Maintain detailed records per MDH and payer guidelines, ensuring accurate billing through proper CPT code usage and modifiers
Partner with the care team to review clinical progress, make adjustments to treatment plans, and ensure optimal patient outcomes
What You'll Need:
Active MN Speech Language Pathologist License
Master's degree in Speech Language Pathology
American Speech-Language-Hearing Association (ASHA) Certificate of Clinical Competence preferred
Experience with seniors, geriatric dysphagia, and home settings, preferred.
CPR certification
Reliable vehicle with valid license and insurance
