Exciting things are happening at Lower Lights Christian Health! We're expanding our mission to serve the community with the opening of our new Northland site. As we get ready for our grand opening January of 2026, we're looking for compassionate and dedicated individuals to join our growing team and make a lasting impact.

Position Summary

We are seeking a dedicated and mission-aligned Advanced Practice Provider to join our integrated care team. The Advanced Practice Provider will provide high-quality, age-appropriate, and specialty-specific care to LLCHC outpatients in coordination with a collaborating physician. The APP will practice within their scope of licensure, certification, and education, and will work alongside a multidisciplinary team to deliver holistic care in alignment with LLCHC's faith-based mission.

Key Responsibilities

Deliver high-quality, patient-centered primary care to individuals and families of all ages.

Manage clinical schedules efficiently while maintaining patient access and continuity of care.

Utilize evidence-based guidelines to support diagnostic and treatment decisions.

Document care in a timely, accurate, and compliant manner within the EHR (Epic experience preferred).

Serve as a clinical leader in your care team and actively participate in team-based care delivery.

Support the implementation of clinical initiatives that improve patient outcomes and health equity.

Participate in quality improvement efforts and contribute to a culture of excellence and accountability.

Work Environment & Physical Requirements

Primarily clinic-based; may include occasional community outreach or telehealth.

Ability to lift up to 25 lbs, bend, stoop, and move intermittently throughout the day.

May require occasional evening or weekend hours based on patient needs.

Medical care will be of the highest quality and will be monitored by peer review and continuous quality improvement.

Qualifications

Required Qualifications

Master's or Doctoral degree in Nursing or Physician Assistant Studies

Board certification in the relevant specialty

2+ years of clinical experience in a healthcare setting

Strong communication and interpersonal skills

Experience with telemedicine services

Current and unrestricted state license to practice as an NP or PA

National certification from recognized certifying bodies (e.g., AANP, ANCC, NCCPA)

Current BLS certification (ACLS and PALS preferred).

Demonstrated commitment to serving diverse, underserved, or vulnerable populations.

Strong interpersonal and communication skills with a team-oriented approach.

Preferred Qualifications

Prior experience in a community health center or similar primary care setting.

Familiarity with EPIC or other EHR systems.

Bilingual (Spanish, Somali, or Arabic) is a plus.
