As an Enhanced Care team Recovery Coordinator, you will be employed by Waythrough to support higher need service users who are unable to progress their timeframe due to their multiple-needs.

Multiple needs include but not limited to; chronic physical health, comorbidities, financial issues, memory/cognitive impairment, age, housing, limited social support and other multi-agency support needs.

This is an interesting and rewarding role where you will have the flexibility to complete greater levels of outreach and go to service users to reduce barriers to engagement, support them to engage with specialist services, bring the intensive support and interventions to the individuals and enhance positive outcomes through this flexible working.
