Synapse Center for Health and Healing is a dynamic, patient-centered functional medicine clinic located in Eagan, MN, dedicated to identifying and addressing the root causes of chronic disease. We utilize an integrative, systems-biology approach to health and wellness, combining advanced diagnostic testing, personalized nutrition, lifestyle modifications, and targeted supplementation. We are seeking a highly motivated and experienced Nurse Practitioner (NP) with specialized training and passion for Functional Medicine to join our collaborative team.

Key Responsibilities

The Functional Medicine Nurse Practitioner will play a crucial role in delivering exceptional, personalized patient care, including:

Comprehensive Patient Evaluation: Conducting thorough initial consultations, including detailed history-taking focused on genetics, environment, and lifestyle factors.

Root Cause Analysis: Utilizing the Functional Medicine Matrix and Timeline to investigate the underlying imbalances contributing to patient symptoms (e.g., gut health, hormonal balance, detoxification, mitochondrial function).

Advanced Testing: Ordering, interpreting, and explaining results from specialized functional laboratory tests (e.g., GI-MAP, organic acids, micronutrient panels, advanced hormone testing).

Personalized Treatment Plans: Developing and managing individualized, evidence-based treatment protocols that may include dietary changes, lifestyle prescriptions, targeted nutraceuticals, and potentially hormone replacement therapy (HRT) where applicable.

Collaboration: Working closely with physicians, nutritionists, health coaches, and other providers within the practice to ensure seamless, holistic patient management.

Documentation: Maintaining accurate, timely, and detailed patient records in compliance with all medical and practice standards.

Patient Education: Serving as a dedicated educator and coach, empowering patients to take ownership of their health journey.

Qualifications & Experience

Required:

Current, unrestricted Nurse Practitioner (NP) license in the state of Minnesota (MN).

Master’s or Doctoral Degree from an accredited Nurse Practitioner program.

Minimum of 2 years of experience as a practicing NP.

Documented training and experience in Functional Medicine (e.g., certification or significant coursework from The Institute for Functional Medicine (IFM), or similar recognized program).

Preferred:

IFM Certified Practitioner (IFMCP)..

Background in areas such as endocrinology, gastroenterology, or integrative/holistic medicine.

Strong knowledge of evidence-based nutraceuticals and dietary protocols (e.g., Paleo, Keto, elimination diets).

Skills & Competencies

A systems-thinking mindset and a passion for complex, chronic health cases.

Ability to synthesize complex diagnostic data into understandable, actionable treatment plans for patients.
