Position Overview: LICSW Care Manager (DC Medicaid - Pediatric Population)

Location:

Washington, DC 20032

Schedule:

Monday - Friday, 8:00 AM - 5:00 PM

Flexible scheduling for field visits (typically 3 field days/week)

Salary:

$90,000 - $98,000 annually

Typical Day-to-Day Responsibilities

As a Care Manager, you will support enrollees in the DC Medicaid system-many of whom experience behavioral health challenges and significant social determinants of health (SDOH) barriers.

Core Responsibilities Include:

Conduct comprehensive needs assessments for assigned enrollees

Assign acuity levels (1-3) to determine care intensity and case management frequency:

Level 1: Well-managed, minimal use of Medicaid resources

Level 2: Moderate needs requiring regular engagement

Level 3: High-utilization, complex care requiring frequent contact

Develop and maintain individualized care plans in partnership with enrollees and their families

Coordinate care across healthcare providers and attend key medical visits (e.g., well-child visits)

Manage a caseload of:

Up to 75 patients across all acuity levels (for standard care managers)

~60 patients at Level 3 acuity (for specialized care managers)

Complete an average of 3 care plans or assessments per week

Conduct approximately 3 field visits per week, with 2 days reserved for documentation

Build and maintain collaborative relationships with enrollees' primary care providers

Training & Onboarding

Initial Training Period (3 weeks):

Comprehensive onboarding to care management systems, policies, and Medicaid contractual obligations

Hybrid training format with increased in-office time for mentorship and team support

Weekly in-person team meetings

Gradual increase in caseload (starting with 30 patients)

Assigned peer preceptor for direct support during onboarding

Qualifications

Required:

Active LICSW (Licensed Independent Clinical Social Worker)

Minimum of 2 years' experience in case management, clinical social work, or a related field

Experience working with pediatric, behavioral health, or Medicaid populations

Preferred:

5+ years of case management or clinical experience

Experience in managed care environments

Familiarity with Medicaid programs and requirements
