Registered Nurse – Home-Based RN

We are currently seeking Registered Nurses interested in transitioning to home-based clinical care. Whether your background is in critical care, telemetry, stepdown, or med-surg, this role offers the opportunity to work independently while remaining part of a collaborative and supportive interdisciplinary team.

What You’ll Do:

Deliver skilled, patient-centered nursing care in the home environment

Conduct assessments, develop individualized care plans, and evaluate outcomes

Collaborate with physicians, social workers, and therapists to coordinate care

Educate patients and families to promote comfort, safety, and independence

Utilize evidence-based practice and technology to support clinical decision-making

Qualifications:

Active RN license in Pennsylvania (New Jersey license a plus)

Minimum 15 months of professional nursing experience (acute care strongly preferred)

BLS/CPR certification (AHA)

Reliable transportation, valid driver’s license, and auto insurance

BSN or ASN from an accredited nursing program
