Job Summary:
Housecall practitioner sees patients in-home and assisted living facilities in central and rural Oklahoma.
Perform full exam encounters
Disease management
Acute follow ups
Urgent encounters prn
Transitional care management
Coordinate with ancillary services to provide additional care (DME providers, home health providers, Medicaid case managers)
Diagnostic review, medication management, and disease teaching of common conditions such as CHF, DMII, COPD, etc
Coordinates with clinical care team support staff regarding care management
Qualifications:
Must be licensed in the state of Oklahoma, APRN or PA preferred. Must hold a current non-restricted prescriptive license from OBNDD and DEA. Oversight physician will be provided, and is actively engaged in care management and case reviews. Family practice or geriatric experience is a must.
Additional Offerings:
Pay is dependent on performance / RVU
Additional compensation awarded for: wound care, new patients, hospital f/u
Monthly bonus available for average visit count (32 per week)

About the Role:
Best Physicians has been operating since 2008, servicing the homebound population of Oklahoma. In 2022, under new owners, the coverage area and services offered by Best Physicians has increased in order to better manage patients in need of housecall visits. The home office is currently located in Chandler, OK; however, most practitioners on staff coordinate care remotely, and conduct daily home visits without office assignment. Providers have an established patient census of approximately 130-150 patients. Patients are generally seen every 4-6 weeks, routinely, to manage chronic conditions, and it is important for practitioners to establish a relationship with patients assigned to their case load. Providers have the option to self-schedule routine follow-up appointments, or may defer to the practice scheduler for all scheduling needs. Clinical support staff is available for clerical needs 08:00am-4:30pm Monday through Friday. A medical scribe is offered for transitional care and new patient encounters. All credentialing is provided by the practice, as well as professional liability insurance.
