Role Summary
The Team Leader, Care Management, oversees and manages the Care Management staff to ensure compliance with service scope, timelines, and program guidelines. This role includes mentoring and coaching team members, reviewing operational performance, and maintaining high standards of quality and professionalism. The Team Leader collaborates with leadership and Learning and Development teams on hiring, onboarding, and performance management while also supporting field-based care coordination and the review of critical incidents to ensure safety and compliance.
Key Responsibilities
• Provide leadership and supervision to the Care Management team, ensuring quality and compliance.
• Coordinate and manage daily operations, including staffing, caseload assignments, and reporting.
• Support team development through coaching, training, and performance evaluations.
• Review and investigate critical incidents to identify root causes and implement corrective actions.
• Escalate critical incidents when individual or staff safety is at risk.
• Ensure timely and accurate documentation and reporting in compliance with policies and regulations.
• Develop and implement strategies to enhance patient safety and service quality.
• Conduct field visits to evaluate staff performance and ensure adherence to standards.
• Participate in IDT meetings and transition planning to support care continuity.
• Attend Fair Hearings with Care Managers as needed during the appeals process.
• Facilitate regular team meetings to discuss cases, share best practices, and resolve issues.
• Lead quality improvement initiatives using data to identify opportunities for process enhancements.
• Monitor key performance indicators and provide performance reports to senior management.
• Collaborate with other departments and external providers to ensure coordinated, effective service delivery.
Required Qualifications
• Associate's or Bachelor's degree in Nursing from an accredited institution.
• Current and valid Registered Nurse (RN) license.
• Minimum of 5 years of nursing experience, with at least 2 years in a leadership or management role.
• Minimum of 2 years of field-based clinical experience in a hospital, long-term care, subacute rehab, or home health setting preferred.
• Experience working with individuals with disabilities, complex medical or behavioral needs, or chronic illnesses.
• Strong understanding of care management principles and practices.
• Knowledge of Medicaid, Medicare, and/or state waiver programs preferred.
• Proficiency in electronic health records (EHR) and Microsoft Office applications.
• Valid driver's license, automobile insurance, and reliable transportation for frequent travel.
