Creekside Care Center of Gahanna is seeking a dedicated PRN SLP/CCC to join our compassionate, resident-focused rehab team! This flexible PRN position is ideal for a clinician who values high-quality care, strong teamwork, and the ability to pick up shifts that fit their schedule.

Responsibilities

Evaluate and treat residents with communication, cognitive, and swallowing disorders

Develop and implement individualized treatment plans

Provide education to patients, caregivers, and families

Document services accurately and timely per company, state, and federal guidelines

Collaborate closely with therapists, nursing staff, and the interdisciplinary team

Deliver patient-centered care that supports safety, function, and overall quality of life
