This role provides primary care inside a skilled nursing facility under the ISNP program, managing a consistent panel and focusing on keeping residents in place and out of the hospital.

Compensation

Base Salary: ~$120,000 + $4,000 annual bonus

Per-visit or additional bonus structures may be discussed as census grows — higher earning potential with established patient panel.

What You’ll Do

Provide longitudinal primary care to ~75–80 long-term care residents per month (panel-based)

Round onsite 4 days per week between two facilities (typically Warren & Marienville)

One remote admin day weekly

Manage both chronic and acute conditions

Focus on “treat-in-place” care models to prevent unnecessary hospitalizations

Coordinate with facility staff and internal medical leadership as needed

Requirements

Board-Certified Physician Assistant licensed in Pennsylvania

Experience with adult/geriatric populations in primary care, post-acute, or SNF settings preferred

Comfortable working independently inside facilities
