Description
Bridges Liaison
Compensation: $42,000/year
Bridges Liaisons with the National Youth Advocate Program work in the community as part of an integral service team. In this role, you will provide support and guidance to young adults eligible for the state administered Bridges Program as they transition out of foster care. Under the general supervision of the Bridges Supervisor or designee, you will work closely with young adults and community partners, providing advocacy and support toward the ultimate goal of living safely and successfully in the community.

Responsibilities
• Facilitates eligibility and enrollment into the Bridges Program.
• Completes face-to-face home visits and assessments.
• Ensures ongoing Bridges eligibility requirements are met and maintained.
• Completes goal setting through the development of the Bridges Plan.
• Identifies, initiates, and coordinates housing referrals and case management needs.
• Provides independent living skills education.
• Supports post-secondary school enrollment, vocational program enrollment, or securing employment.
• Connects participants with physical & mental health care and community resources for self-sufficiency.
• Engages with participants to develop a trusting, supportive relationship while assisting with achieving identified goals.
• Completes ongoing documentation to track and report case management activities.
Minimum Qualifications
• Bachelor’s degree in social work, human services, behavioral health or related field.
• Experience working with families in the child welfare system.
• Daily travel required with the willingness to meet participants within 1.5-hour radius, on a flexible schedule or during non-traditional hours if needed.
• Must be comfortable providing services both in the community and in the home.
• Proficient use of desktop and laptop computers, internet search, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
• Must be willing to perform other duties as assigned.
Driving and Vehicle Requirements
Valid driver’s license
Reliable personal transportation
Good driving record
Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
