Description

Conduct comprehensive evaluations of patients in both inpatient and outpatient settings while creating personalized treatment plans

Deliver physical therapy interventions concentrating on enhancing mobility, strength, balance, and overall function

Engage in collaboration with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes

Inform and educate patients and families about therapeutic objectives, discharge procedures, and necessary home exercises

Maintain precise documentation of evaluations, treatments, and progress following facility and regulatory standards

Contribute actively to care team discussions and interdisciplinary conferences to ensure united and coordinated care Education

Doctorate in Physical Therapy (DPT) or an equivalent credential from an accredited institution Certifications

State licensure as a Physical Therapist

Preferred CPR certification

Specialized certifications (such as neurological or geriatric) are advantageous, but not required Skills

Previous experience is strongly preferred

Aptitude for treating patients with intricate medical and mobility challenges
