The Inpatient Support Center (ISC) RN provides clinical support and maintains a global view for hospitalized COPC patients. This nurse serves as a comprehensive resource that provides support to COPC Hospitalists and outpatient providers, hospital care teams, and community partners. Professional collaboration with covering providers, critical thinking, and a well-rounded experience in the acute healthcare setting are essential.
Responsibilities:
Act as a change agent for innovative approaches to healthcare; consistent with active participation in COPC initiatives to advocate for patient-centricity in everything that we do.
Display consistent professional conduct and interpersonal skills.
Provide clinical support, facilitate movement, and maintain a global view for hospitalized COPC patients.
Demonstrate high clinical expertise for acute care and competence in utilizing the nursing process.
Display the ability to thoroughly triage complex patient issues, utilize high-level assessment skills, and provide support to the hospital care teams and Hospitalist providers.
Differentiate and prioritize routine and urgent needs, recognize the gravity of a particular health care problem, and consult with a medical provider as needed.
Maintain knowledge of current processes, operations, and COPC outpatient resources.
Provide prompt and effective communication with the hospital ED and consulting providers, hospital transfer centers, PCP Offices, and outside partners when facilitating patient admissions, tracking and clinical needs.
Enable continuity of care and efficiency by maintaining the Hospitalist's patient lists and serve as a well-informed resource.
Provide a complete and comprehensive report to incoming RN and providers.
Work effectively with physician supervision.
Other duties as assigned. Requirements:
Current Ohio Registered Nurse (RN) License, BSN preferred
Minimum 3 years acute care nursing in a hospital setting
Experience with EPIC electronic health record system
Self-disciplined, energetic, passionate, and innovative
Must be detail-oriented and highly organized
Ability to communicate clearly and effectively with patients and other external parties in a courteous and friendly manner
Ability to maintain confidentiality at all times
Ability to react calmly and effectively in emergency situations
Ability to interpret, adapt, and apply guidelines and procedures
Ability to communicate clearly and document efficiently in EHR
Knowledge and understanding of medical terminology
Skill in assisting in a variety of treatments and medications, as directed
Skill in developing and maintaining department quality assurance
Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public
Customer service skills; social perceptiveness and service oriented
Excellent time management skills and ability to multi-task and prioritize work
Knowledgeable in all areas of acute adult care
Knowledgeable of hospital departments
Competent in recognizing the gravity of a particular health problem, able to prioritize those needs and act upon urgent medical problems while providing safe, effective and appropriate care.
