About the Role

As a Nurse Practitioner with Matrix, you will conduct thorough health assessments during visits that last up to one hour. You will delve into the member's health history, medication use, social factors, and more to gain valuable insights that are often missed in regular office visits. Your contributions will help improve quality of care and address multiple health concerns in a single visit. Your key responsibilities include:

Perform comprehensive adult and geriatric health assessments, including documenting medical history, diagnoses, treatments, and follow-up recommendations in line with Matrix's protocols.

Administer and order necessary point of care and lab-based tests.

Collaborate closely with physicians, case managers, social workers, and other healthcare professionals to provide holistic care.

Work with Primary Care Physicians on patient education and follow-up as required.

Deliver care in various settings, including members' homes and Skilled Nursing Facilities.

Qualifications

Master's Degree in Nursing or equivalent experience with a valid Nurse Practitioner license.

Current NP licensure in your state, including the ability to prescribe medications.

AANP, AACN, or ANCC board certification in relevant nursing specialties.

Certification in BLS, ACLS, or CPR.

Minimum of one year of experience as a Nurse Practitioner.

Willingness to travel up to 80-90% of the time, including overnight and weekend travel across the state and possibly adjacent states.

Valid driver's license and adequate auto insurance.

Strong computer and electronic health record software skills.

Excellent communication skills, both verbal and written.
