Position Summary:

Under the leadership of the AVP, Quality, the Nurse Practitioner (NP) is an active member of the CRHS team that delivers medical care consistent with the strategic vision, goals, philosophy, and direction of CRHS. The Nurse Practitioner (NP) is responsible for providing care to patients throughout their life cycles (prenatal, pediatric, adolescent, adult, and geriatric) while emphasizing wound care and prevention, and ostomy services. Acts as the clinical resource, educator, and consultant to the staff in the development and implementation of assessment and treatment using established procedures and guidelines, Recommends preventive measures, wound care products and adjunct therapy.

Basic Qualifications:

Education:

Requires a Master’s Degree in Nursing

Experience:

Requires two to four years of work-related experience or any equivalent combination of education, training, and experience.

Licensure, Registrations & Certifications:

Requires and maintains current state license as a Registered Nurse

Requires and maintains current state certification as a Nurse Practitioner (NP)

Maintains a current certification in Basic Life Support (if BLS verification is not current upon hire, it must be obtained within three (3) months of hire)

Preferred certification in wound care

Essential Job Responsibilities:

Development of CRHS Wound Care Program, including Policies, Procedures and Protocols

Eliminate risk and external mechanical factors, implementing frequent skin assessment and early skin care intervention to prevent the formation of pressure ulcers and promote an optimal wound bed necessary for healing.

Utilize an individualized plan of current methods and modalities to direct daily care for maintaining skin integrity, healing existing wounds, and promoting comfort.

Reduce the incidence of pressure wounds in the at-risk population.

Communication among the team to ensure that the patient is receiving care that is comprehensive, timely and follows established treatment guidelines.

Coordinates efforts in wound care across the CRHS continuum

Eliminates duplication of services, enhancing patient/family understanding and compliance and increasing satisfaction among the patient/family and staff

Staff training and education on wound care

Direct oversight of inpatient and nursing home wound care

Provides direct patient clinical care in the inpatient, nursing home, and ambulatory setting.

Rounds on patients at CRH and CRH Nursing homes and is available for consultations.

Provides appropriate level of clinical and educational services as needed, that is age and condition appropriate.

Participates in performance improvement (PI) projects.

Provides assistance to physicians on epidemiological issues.

Maintains chart documentation, tracking and reporting systems as indicated for continuity of care.

Follows all Physician Services, CRHS, and Healogics policies and procedures and ensures compliance with all federal and state laws, regulations, and JCAHO requirements.

Mentors’ new employees as requested.

Completes all required professional education including continuing nursing education hours and mandatory in-services to maintain professional licensure.

Performs other related job duties as assigned.
