Job Overview

Position: Physical Therapist – Home Health

Facility: VNA Hospice of the Southwest Region (St. Albans)

Address: 8 N Main Street, St. Albans, VT 05478

Start Date: 12/10/2025

Contract: 13 weeks

Shift: Monday–Friday, 8a–4p (No call, No holidays)

Guaranteed Hours: 40 hrs/week

Mileage Reimbursement: $0.625/mile

Requirements

Active VT PT license (or willing to obtain upon acceptance)

Minimum 2 years of recent Physical Therapy experience

Home Health experience required

Proficiency with McKesson EMR preferred

Strong documentation and patient care coordination skills

Must be able to work independently and manage patient caseload efficiently

Responsibilities

Conduct comprehensive OASIS & PT assessments

Develop individualized care plans to promote functional independence

Provide therapeutic treatments and patient/family education

Supervise PTAs and coordinate care with the interdisciplinary team

Complete all documentation, billing, and visit reports per state/federal standards
