Summary, Job Description, and Qualifications

Job Title: Community Resource Coordinator

Reports To: Associate Director of Case Management

Direct Reports: None

Days: Monday – Friday; some weekends may be assigned

FLSA Status: Exempt, Administrative

Summary:

Applying best practices in child welfare, the Community Resource Coordinator is responsible for cultivating flexible, family-focused, community-based responsive services for the purpose of assuring safety and permanency for families. This role involves providing direct care coordination services to families, assessing safety in the home environment, facilitating Family Team meetings, linking families to case plan services, evaluating the effectiveness of those services and ensuring collaboration among the service team, service providers, natural supports and community resources.

Essential Tasks, Duties, and Responsibilities:

Safety Monitoring: Initiate contact with children and families in the home setting a minimum of 2 times per month; Observe family dynamics including parenting practices, routines, and relational interactions. Monitor and assess safety concerns at each visit; Monitor compliance with safety plans when applicable and share observations with service team. Serve as a mandated reporter.

Service Coordination and Monitoring: Facilitate referrals and linkages to appropriate services and supports; Monitor the child and family centered case plan to ensure services are accessed; Coordinate communication among providers and organizations to assure a broad picture of case plan progress; Obtain monthly progress reports from providers and review highlights with the service team; Convene and facilitate family team meetings with family, service team and community providers at least 1 time per month; Serve on OhioRISE Child and Family Teams as directed.

Advocacy and Family Support: Advocate for families’ needs including financial, housing, medical, legal, social, and environmental support; Assist families in accessing public entitlements and maintaining benefits; Provide education and training to enhance families’ self-sufficiency and daily living skills; Encourage family-driven and youth guided planning and decision making; Gather information to access funds for services and items as indicated by the case plan; Attend Semi-Annual Reviews and Annual Court Reviews with assigned families.

Transportation: When necessary, coordinate and/or provide transportation for families to attend appointments that align with case plan goals and required services

Availability: Employment hours are flexible and need to align with the availability of the family unit. The position requires the employee to be available at least 2 evenings each work week until 7:00 pm. When necessary to meet the availability needs of a family, weekend hours may be scheduled at the discretion of the supervisor.

Documentation and Compliance: Complete activity log documentation within specified timelines; Provide documentation justifying legal status as requested; Maintain compliance with organizational policies and legal standards.

Caseload: Maintain a caseload of 25 families;

Training and Development: Participate in required orientation and ongoing training topics relevant to child welfare; Attend staff meetings and individual supervisory conferences as scheduled;

Other duties as assigned.

Knowledge, Skills, and Abilities:

Bachelor’s Degree with a minimum of one year of professional experience working in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related human services field; Bachelor's of Social Work desired.

Experience with community services, family systems, and behavioral health

Be culturally competent or responsive with training and experience necessary to manage complex cases.

Ability to use a computer; proficiency in Word and Electronic Health Record (EHR).

Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance.

Willingness to travel for various reasons, mainly during the day but occasionally overnight. Ability to visit clients’ homes (may or may not be handicapped accessible).

Excellent verbal and written communication skills; strong teamwork and organization/time management skills.

Maintain a HIPPA compliant home-based office space if/when remote work is approved

Physical Demands:

Occasionally move about inside the office to access file cabinets, office machinery, etc.

Constantly operates a computer and other office productivity machinery, such as a copy machine, computer printer, etc.

Ability to communicate (oral and written) with all levels of personnel, internal and external to the company

Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.

Must be able to lift 20 pounds independently
