Job Description

Responsibilities

Conduct initial and ongoing patient assessments utilizing OASIS documentation.

Develop individualized care plans in collaboration with physicians, therapists, and other healthcare team members.

Provide direct skilled nursing care and patient education in the home setting.

Monitor and evaluate patient progress, updating care plans as needed.

Ensure timely, accurate, and compliant documentation.

Communicate effectively with patients, families, and the interdisciplinary team.

Uphold agency standards and regulatory requirements.

Qualifications

Active RN license in the state of [State].

Minimum 1 year of OASIS documentation experience required.

Home Health experience strongly preferred.

Compassionate, patient-focused, and adaptable to a dynamic work environment.

Strong communication, clinical assessment, and time management skills.
