Job Description

American Traveler is seeking an experienced Physical Therapist for a Home Health contract role requiring recent Oasis and HCHB experience.

Responsibilities

Home health setting with a territory up to a 60-mile radius from the main office

Geriatric patient population with an average caseload of 5-7 patients per day

Day shift schedule: 8:00 AM to 5:00 PM, five 8-hour shifts per week

On-call rotation required every 3rd to 4th weekend

Uses HCHB (Homecare Homebase) EMR; proficiency is required

Oasis documentation required for SOC (Start of Care) visits

Candidates must wear scrubs in any color; jeans are not permitted

Travelers will be provided with an iPad for documentation and mileage is reimbursed at the IRS standard rate

Strong emphasis on timely and accurate record keeping and professional communication

Applicants must be comfortable with independent work and managing daily scheduling

Local candidates are accepted at the travel rate

RTO (requested time off) must be disclosed at the time of application

Requirements

Active Physical Therapy license required if applicable

Minimum 1 year of recent home health experience required

Proficient Oasis and HCHB experience required

Current BLS certification required

Flexibility with coverage area is required

Timely documentation and communication are essential
