Job Title: Temp – Physical Therapist – Long Term Care (Days)

Location: Groveton, TX

Assignment Type: Temporary (Contract)

Position Overview

We are seeking a skilled and compassionate Physical Therapist to join our team on a temporary basis in a Long Term Care (LTC) setting. The Physical Therapist will assess, diagnose, and treat patients to restore mobility, reduce pain, improve function, and prevent disability. This role focuses on individualized care plans to enhance the quality of life for patients in a supportive LTC environment.

Key Responsibilities

Conduct comprehensive patient evaluations, including history-taking, systems reviews, and administering tests and measures to identify movement and functional deficits.

Develop and implement individualized treatment plans with patient-centered goals in accordance with physician orders.

Monitor and reassess patient progress, adjusting treatment plans as needed.

Initiate and manage the discharge planning process from the first visit through completion of treatment.

Supervise and delegate tasks to ancillary staff and students in compliance with state regulations and department policies.

Complete accurate, legible, and timely documentation in accordance with policies, accreditation standards, and insurance requirements.

Communicate effectively with patients, families, caregivers, and the healthcare team to maximize treatment outcomes.

Provide education and guidance to patients, families, and caregivers to promote understanding and adherence to therapy plans.

Participate in professional development through in-service training, workshops, and continued learning.

Contribute to quality improvement initiatives and maintain a safe environment for patients, visitors, and staff.

Adhere to all safety standards and operate within the defined scope of practice as established by the state’s practice act.

Perform additional duties as assigned.

Qualifications

Current, valid Physical Therapist license in the state of Texas.

Strong clinical skills in assessment, treatment planning, and rehabilitation practices.

Excellent communication, collaboration, and documentation abilities.

Commitment to patient-centered care with a focus on quality and safety.

Experience in a Long Term Care or rehabilitation setting preferred.

Schedule

Day shifts in a long-term care setting.

Temporary assignment with the potential for extension.
