Your Role and Responsibilities as a Case Manager

Collaborate with an interdisciplinary team to guide treatment plans tailored to each patient's needs.

Establish tentative discharge plans in coordination with team members.

Participate in planning and executing a smooth discharge experience for patients.

Monitor patient experiences, ensuring quality, service appropriateness, and alignment with payor expectations.

Facilitate weekly team conferences to coordinate treatment plan adjustments.

Document comprehensive case management addendums and necessary reports.

Stay updated on regulations, policies, and procedures relevant to the role.

Analyze case management reports to proactively implement necessary actions.

Engage in Acute Care Transfer meetings to identify trends and devise solutions.

Meet with patients and families within 24 hours of admission as per standards.

Complete assessment addendums within 48 hours of patient admission.

Educate patients and families on rehabilitation and the role of the Case Manager.

Coordinate family conferences when necessary.

Assist patients in procuring timely resources to avoid discharge delays.

Ensure compliance with regulations regarding orthotics and prosthetics ordering.

Make timely referrals to post-discharge providers and ensure documentation is thorough.

Verify accuracy of discharge information in patient records.

Engage in utilization review processes, including data collection and trend analysis.

Participate in on-call case management duties as required.

Qualifications You Need

License or Certification Required:

Qualified to independently complete assessments within your professional scope (e.g., RN, SW, OT, PT, ST, and Rehabilitation Counseling).

Must hold an active license if required by the state.

Eligibility for CCM® or ACM certification upon entering this role, or within two years.

CCM® or ACM certification required or must be obtained within two years of achieving the Case Manager II position.

Minimum Qualifications:

For nursing candidates, a minimum of an Associate Degree in Nursing with RN licensure; BSN preferred.

For all other eligible licensed or certified health care professionals, a minimum of a bachelor’s degree; graduate degree preferred.

Preferably, 2 years of rehabilitation experience.
