Job Description
American Traveler is seeking a Physical Therapist for a Home Health hospice assignment requiring at least 1 year of recent experience and BLS certification. Responsibilities
Work in a home health hospice setting covering a territory up to 60 miles from the hub
Patient population is primarily geriatric
Average caseload is 5-7 patients per day, which should be confirmed during interview
Day shifts scheduled Monday through Friday from 8:00 to 17:00
On-call rotation required every 6th weekend
Uses HCHB EMR; experience required
An iPad will be provided for documentation
Mileage reimbursement is provided at the IRS standard rate
Responsibilities include providing hospice PT services in a home health environment and maintaining timely, accurate documentation
Must participate in on-call rotation as assigned
No first-time travelers will be considered
Scrubs of any color are acceptable; jeans are not permitted
A Hospice/Home Health checklist is required for consideration
No call-ins are permitted for this position
All required and requested time off must be specified prior to assignment consideration Requirements
Current PT license required if applicable
Minimum of 1 year recent experience as a Physical Therapist
BLS certification required
Experience with HCHB EMR is required
OASIS experience required for those completing Start of Care visits
Flexibility in covering the assigned territory is expected
Documentation and communication skills must be timely and proficient
