SALMON Home Care is seeking a full-time Home Health RN to deliver personalized care and build meaningful relationships with patients in the Natick/Framingham area.

What You'll Do as a Home Health RNProvide skilled nursing care to patients in their homes * Complete initial assessments and develop individualized care plans * Administer medications, IV therapy, wound care, and other clinical treatments * Educate patients and caregivers on managing chronic conditions, medication use, diet, and post-acute care * Coordinate care with physicians, case managers, and the broader home health team * Ensure timely and accurate clinical documentation Hiring Locations:Natick / Framingham Full-time hours (32-40/week) with flexible AM start times to support your lifestyle and scheduling needs.
