Hospice Nursing Job Details

Location: Columbus, Ohio (serving Columbus and surrounding areas)

Assignment Type: Travel RN, On-Site

Contract Length: 13-week travel nursing contract

Start Date: November 17, 2025

End Date: February 15, 2026

Shift: Monday – Friday, 8:00 AM – 5:00 PM

Shift Duration: 8-hour day shifts

Hours per Week: 40

Estimated Weekly Pay: $1900 – $1950

EMR System: HCHB (Homecare Homebase)

RN Case Manager Qualifications

Active Registered Nurse (RN) license in Ohio

Minimum 12 months of recent hospice nursing experience within the past 3 years

Experience with HCHB EMR system preferred

Strong clinical, communication, and organizational skills

Ability to work independently and manage a hospice caseload

RN Case Manager Responsibilities

Coordinate and manage hospice care for patients in Columbus, Ohio and surrounding areas

Develop, implement, and evaluate individualized patient care plans for hospice patients

Provide compassionate support, education, and advocacy for patients and families

Collaborate with interdisciplinary healthcare professionals to ensure high-quality hospice care

Accurately document patient care and progress using HCHB EMR system
