Job Summary
This is a full-time professional position. This position will assist in providing Palliative Care Consultation to patients in the hospital setting. This position will provide direct patient care on an ongoing basis. This position will work with the Director of Inpatient Palliative Medicine to educate hospital staff and community physicians about palliative care, pain and symptom management, communication around end of life decision making, and other programs available within Transitions LifeCare. The schedule for this position is Monday-Friday 8:30am-5:00pm; weekends and holidays as needed/scheduled.
Responsibilities what you do
Provide Palliative Care Consultation to patients in the hospital.
Provide direct patient care on an ongoing basis as directed by and under the clinical supervision of the Director of Inpatient Palliative Medicine; utilizing pain and symptom management interventions that are based on established hospice and/or palliative care guidelines and standards.
Attend IDG meetings as appropriate
Document in patient medical record to meet federal, state, regulatory, and agency guidelines.
Educate hospital staff and consult with the community physicians about palliative care pain, and symptom management, communication around end of life decision making and other Transitions LifeCare Programs.
Assist in data collection, forms, and outcome evaluation tools for the PalliativeCare Program as needed.
Participate in on-call rotation
Perform other duties as assigned by supervisor.

Requirements what you need
Nurse Practitioner with current N.C. license
Preferred 3 years clinical experience.
Preferred 2 years’ experience in hospice, palliative care and/or home health
Preferred ACHPN certification
Maintain educational requirements to meet Nurse Practitioner standards.
Knowledgeable about reimbursements sources, Medicare regulations, licensing laws, and accreditation standards.
Computer skills preferred
Current CPR certification.
Must have and maintain a valid North Carolina Driver’s License, maintain automobile insurance coverage and have access to reliable transportation.
Full-time
