Call required as needed

Every third weekend

Must agree to work a minimum of 1 major holiday

1st time travelers accepted

Required: BLS & WI State or compact lic

In-home visits required - Service areas is a 40 mile radius - Aim to keep staff within the same area for assignments - Required to cover patient in community based retirement homes within Beaver Dam

Facility reimbursed mileage based on the IRS policy - $0.70 per mile1-2

Contractor
