Job Summary
The role of the Licensed Practical Nurse is to work as a member of the primary health care team in a supportive capacity to the Physician and Nurse Practitioners.
Qualifications and Requirements
License: Current Florida LPN license and certification to administer medication.
Education: Graduation from an accredited LPN program.
Ability to use the nursing process, as evidenced by physical and psychosocial observation skills, interviewing techniques, communication, and interpersonal skills, problem-solving, and clinical judgment. Maintain patient confidentiality. Spanish fluency is desirable, but not essential.

Demonstrates knowledge and full understanding of HIPAA patient privacy rights. Maintains confidentiality of all medical, financial, and other sensitive materials and information in printed, electronic, or verbal form, which may jeopardize the privacy of patients and/or employees. Accesses and uses the minimum necessary patient identifiable information to perform job responsibilities and duties and only for authorized purposes.
Work Environment:
The nurse works in a medical clinic environment, which is generally indoors in a climate-controlled environment. The working environment may change somewhat between locations. Nurses will be assigned a home location. However, location assignments will change from time to time, and the nurse must be able to cover at any location, as assigned, either temporarily or on a long-term basis.
Essential Functions
Greets patients and visitors in a friendly, professional manner
Understands that our patients are our priority and maintains a respectful, pleasant and polite demeanor at all times
Resolves patient issues as appropriate - notifies appropriate supervisor if unresolved
Measures and documents patient's primary problems, including vital signs, basic labs, psycho-social history and medications in order to prepare the patient for examination by the provider, anticipating the need for required supplies and equipment
Reports to the provider any significant social or other data obtained during conversation with patient or his/her family
Manages nurse schedule and sees that patient's are notified if appointments are cancelled or modified to avoid patient abandonment
Performs general nursing care to patients. Administers prescribed medications and treatments in accordance with nursing standards of practice and established scope of practice for licensed practical nurses and as prescribed by physician or nurse practitioner
Observes, records, and reports patient's condition and reaction to drugs and treatments to physicians
Attaches lab and radiology results to patient chart for provider review and notifies provider of test results outside abnormal parameters
Assists medical assistants in the completion of medication prior authorizations and sees that they are completed in a timely manner
Recognizes signs/symptoms and/or patient changes and notifies Provider
Reviews and upload temperature logs on vaccination refrigerators and freezers to maintain compliance with Florida Shots, identifies and reports any excursions promptly
Places orders, monitors inventory for vaccinations, monitors vaccine expirations, vaccine compliance through the Florida Shots online system based on vaccination usage and inventory on hand
Inventories, orders and restocks supplies and vaccines as needed
Creates and documents in care plans and self-management support flow sheets
Utilizes nurse schedule to assess patients, educate patients, and administer vaccinations and/or other medications for nurse visits
Returns patient calls/voicemails and places calls as requested by provider in a timely manner. Documents this process in the electronic medical record
Provides patient education to clients utilizing the electronic medical record
Works as a member of the care team to review medical records for completeness of data prior to the visit
Tracks, records, and scans radiological procedure orders for provider
Actively participates in identification and determination of conditions to ensure high quality care to clients/patients and their families
Attends required meetings and participates in health center work as requested
Administers immunizations per CDC guidelines and documents administration appropriately in the patient's medical record
Conducts care team huddles daily for efficient communication among the team, assigns back-up in absence
Participates in Population Health Management activities
Performs phlebotomy/lab tests according to lab policy and procedures
Maintains quality and aseptic techniques using standard policies and procedures
Performs testing/procedures per provider orders such as EKGs, ear irrigation, dressings, ace wraps, nebulizer treatments, and assists Medical Assistants when needed
Performs quality control tests per established protocol
Washes and prepares instruments for sterilization
Demonstrates priority setting and organizational skills
Exhibits punctuality and dependability in attendance and work habits
Performs other related job duties as required by the Chief Nursing Officer

Physical Requirements
This position is very active and requires standing, walking, bending, kneeling, and stooping all day.
This Position Requires employees to handle, or feel objects, tools or controls; reach with hand and arms; climb stairs; balance; kneel, crouch, talk, hear, and smell.
This employee must frequently lift and/or move items over 25 pounds.
Must be able to lift, carry and handle equipment, supplies, and other work site materials according to position requirements.
