MAJOR FUNCTION
Under the direction of the Chief Medical Officer, support patients and caregivers to achieve better management of their asthma, COPD, tobacco dependence and/or other respiratory health conditions through education and care coordination.
ESSENTIAL FUNCTIONS
Instructs patients and families on how to manage their condition, including proper inhaler technique, the action of different medications, and how to prevent relapses.
Develops personalized asthma action plans that outline triggers, current treatments, and what to do in an emergency.
Assesses patient needs, monitor progress, and adapt educational strategies as needed.
Helps patients identify and manage environmental or other triggers that can worsen their asthma/COPD.
Works with Clinical and Administrative leadership and staff to maintain a daily schedule of 6-8
Works with providers, clinical team members and frontline staff to coordinate care.
Serves as a resource for patients, their families, and HJAHC staff regarding asthma management.
Provides individualized and small group teaching and skills training for patients.
Works with Clinical and Quality Improvement staff to identify and implement clinical guidelines and workflows to support excellent patient outcomes for patients with chronic respiratory conditions, including but not limited to meeting UDS measures for tobacco cessation and QI measures for asthma management
Greets all patients/clients/customers via telephone or directly in a personal and professional manner. Promotes positive patient relations. ADDITIONAL RESPONSIBILITIES:
Performs other duties and assumes other responsibilities as apparent and/or as assigned by the Chief Medical Officer. Duties, responsibilities and activities may change, or new ones may be assigned at any time with or without notice.
REQUIREMENTS:
EDUCATION & EXPERIENCE:
Graduation from an accredited College or University with a Bachelor’s degree in Health Education, Nursing, Social Services or related field required
At least two (2) years’ experience in education or counseling in addition to clinical experience
LICENSURE AND/OR CERTIFICATIONS:
Licensed RN or LPN in the State of NJ required
Certified Asthma Educator (AE-C) preferred
KNOWLEDGE, SKILLS, ABILITIES AND OTHER (KSAO’s)
Knowledge of the problems involved in dealing constructively with human relationships, personality disorders, social behavior and social relationships.
Ability to work cooperatively with associates, subordinates, other divisions, branches of the government and the public.
Thorough knowledge of the philosophy of interacting techniques and the program design and procedures used in social service delivery programs.
Ability to prepare clear, technically sound, accurate and informative reports.
Ability to act as the representative of the Center to make presentations as may be required, and to attend and participate in professional and other meetings and conferences.
Computer literacy and proficiency.

This position requires manual dexterity sufficient to operate phones, computers, and other office equipment. The position requires the physical ability to kneel, bend, and perform light lifting. This person must have the ability to write and speak clearly using the English language to convey information and be able to hear at normal speaking levels both in person and over the telephone. Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. The working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials.
