Responsibilities
Manage patient care plans and coordinate services for optimal outcomes.
Conduct assessments to identify patient needs and develop care strategies.
Collaborate with healthcare teams to ensure comprehensive patient support.
Participate in ongoing professional development and training opportunities.
Implement strategies to achieve performance targets and goals.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Document patient interactions and maintain accurate records.
Utilize data to track performance metrics and improve service delivery.
Engage in regular communication with patients to ensure satisfaction.
Provide support and guidance to junior staff and interns.
Stay updated on industry trends and best practices in case management.
Experience/Qualifications
Current RN license in North Carolina and relevant certifications.
Willingness to participate in ongoing professional development and training.
Proven experience in case management or similar healthcare roles.
Ability to develop and implement effective patient care plans.
Commitment to achieving performance targets and goals in case management.
Strong communication skills to engage with patients and healthcare teams.
Experience in monitoring patient progress and adjusting care strategies.
Proficiency in using healthcare data systems for tracking metrics.
Ability to educate patients and families on health management effectively.
Strong organizational skills to manage multiple patient care plans.
