Cypress Cove Care Center is seeking a passionate and skilled Physical Therapist (PT) to join our interdisciplinary rehabilitation team. In this rewarding role, you will provide individualized therapy services to residents recovering from illness, injury, or surgery in our subacute rehab unit.

Key Responsibilities:

Perform comprehensive evaluations of patients' physical conditions and mobility needs.

Develop and implement individualized treatment plans to improve strength, balance, mobility, and function.

Provide therapeutic interventions aimed at restoring independence and preventing complications.

Collaborate closely with occupational therapists, speech-language pathologists, nursing staff, and physicians to ensure coordinated care.

Monitor and document patient progress, update care plans, and participate in discharge planning.

Educate patients and caregivers on home exercise programs and safety strategies.

Adhere to all regulatory, clinical, and facility guidelines and standards.

Qualifications:

Current Florida licensure as a Physical Therapist (or eligibility).

Strong clinical, communication, and organizational skills.

A team-oriented mindset with a compassionate and patient-centered approach.

Experience in a subacute or skilled nursing facility is a plus, but not required.
