Schedule: Monday through Friday, 6:00 AM – 2:30 PM

We are seeking a dedicated and compassionate Case Manager to join our Goldman Clinic team and make a lasting difference in the lives of individuals pursuing recovery.Position Overview
The Case Manager provides case coordination, advocacy, and chart management support to clients enrolled in our Methadone Maintenance Program. This role ensures continuity of care and helps clients access essential community and social services.
You will work directly with clients, counselors, and interdisciplinary team members to promote recovery, retention, and compliance with all regulatory and program standards.Key Responsibilities
• Conduct client assessments and develop individualized service plans
• Coordinate with social service agencies for housing, benefits, and employment support
• Assist clients with Social Security and Department of Welfare documentation
• Monitor client progress and maintain accurate records in the Electronic Health Record (EHR)
• Lead weekly orientation groups to educate new clients on program structure and expectations
• Track funding authorizations and verify insurance coverage
• Identify at-risk clients and implement retention strategies
• Collaborate with clinical staff to ensure seamless treatment coordination
• Provide crisis intervention and referrals when necessary
• Maintain compliance with confidentiality, licensure, and accreditation requirementsQualifications
Education:
• Associate degree or higher in Human Services, Psychology, Social Work, or a related field required
Experience:
• Previous experience in behavioral health, substance abuse treatment, or case management preferred
• Knowledge of community resources, social service systems, and public insurance programs
Certifications:
• Current First Aid and CPR certification preferred (must be hands-on course)
• Bilingual in English/Spanish is a plus
Skills & Abilities:
• Strong communication, organization, and documentation skills
• Working knowledge of psychiatric disabilities, substance use disorders, and recovery approaches
• Ability to multi-task, prioritize, and manage a client caseload
• Experience using Electronic Health Records (EHR) systems
• Team-oriented approach with strong interpersonal and problem-solving skillsWork Environment
• Primarily onsite within a fast-paced clinical setting
• Frequent client interaction, standing, and walking required
• May involve assisting clients in crisis or high-stress situations
