RN CASE MANAGER - HOME HEALTH AND HOSPICE DETAILS AND COMPENSATION:
Location: Yazoo City MS 39194 – 100% On-site
Payrate: $56.14/hr
Required Availability: Full-Time Monday – Friday, 8:00 AM – 4:30 PM
On-call rotation required (approximately every 3 weeks).
RN CASE MANAGER - HOME HEALTH AND HOSPICE SUMMARY AND HIGHLIGHTS:
The RN Case Manager role involves delivering coordinated, patient-centered care through comprehensive assessments, collaboration with providers, and proactive care management.
This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term insurance
A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN CASE MANAGER - HOME HEALTH AND HOSPICE RESPONSIBILITIES:
Conduct comprehensive assessments for high-risk members.
Develop and implement individualized, cost-effective care plans in collaboration with primary care providers.
Coordinate transitions of care with physicians, social workers, and discharge planners.
Monitor outcomes and continuously reassess appropriate levels of care.
Identify opportunities for health promotion and illness prevention.
Provide patient advocacy, ensuring confidentiality and compassionate care.
Manage OASIS documentation and perform wound care, Foley catheter care, IV administration, and patient teaching.
RN CASE MANAGER - HOME HEALTH AND HOSPICE REQUIREMENTS:
Education: H Associate or Bachelor’s degree in Nursing or related field.
Attendance: Commitment to mandatory attendance during the first 90 days.
Active RN license in MS (compact licenses accepted).
CPR certification required.
RN home care experience with HCHB (HomeCare HomeBase) strongly .
Ability to independently complete OASIS documentation and provide direct clinical interventions.
Case Management certification .
On-call rotation required (approximately every 3 weeks).
