As a Licensed Practical Nurse for Homeland HomeHealth, you would contribute to our community of caring by providing direct support to patients where they feel most comfortable, promoting and fostering their healing process.
Homeland HomeHealth is a fast-growing provider of in-home healthcare that offers convenient and effective skilled nursing care in the comfort of our patient’s own home. Using compassion and skill, our nurses provide physician-ordered medical treatment to promote healing, independence, and the best possible quality of life. We believe in a multiple-disciplined approach to help patients in their healing journey, which includes wound care, cardiac care, palliative care, fall prevention, rehabilitation services, intravenous therapy, and other care services.
This is a Full-time position, Monday through Friday, 8:00 am to 5:00 pm.
This position requires a valid Pennsylvania Licensed Practical Nurse license, a valid Pennsylvania driver’s license, and reliable internet access.
As Homeland HomeHealth’s LPN you would provide skilled nursing care to the Home Health patient as directed by the supervising Registered Nurse (RN) and Director of Home Health, in compliance with Medicare Home Health Conditions of Participation, Pennsylvania licensure laws and agency policies and procedures. Provide timely documentation of patient services based on the patient’s plan of care. Work closely with other members of the Home Health Interdisciplinary Team to ensure all patient and family care needs are being met.

Education and Experience Requirements:
Graduate of an accredited practical nursing program.
Minimum age requirement of 18.
Minimum of one year of direct patient care experience in an institutional or home care setting required, with community health/ home care experience preferred.
Ability to work within an interdisciplinary setting.
Ability to work independently.
Licensure, Registration, and Certification Requirements:
Valid Pennsylvania Licensed Practical Nurse license.
Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania state requirements and agency policy.
Satisfactory references from employers and/or professional peers.
Satisfactory criminal background check.
Maintains current CPR certification.
Access to reliable internet access.
