Minimum Qualifications:

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an APTA accredited institution.

Credentials evaluated by one of the following standard organizations: FCCPT, ICD, ICA, IERF, or IEC.

Current state license in good standing with the State License Board.
