Brooksville Healthcare Center is seeking a compassionate and motivated Per Diem Physical Therapist (PT) to join our in-house rehabilitation team. Our facility offers subacute and long-term care rehabilitation, focusing on helping patients regain independence and improve their quality of life.

Responsibilities:

Evaluate patients and develop individualized physical therapy treatment plans.

Provide therapeutic interventions to improve mobility, strength, balance, and functional abilities.

Monitor patient progress and adjust treatment as needed.

Collaborate closely with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians.

Maintain accurate and timely clinical documentation.

Educate patients, families, and caregivers on home exercises and rehabilitation goals.

Qualifications:

Graduate of an accredited Physical Therapy program.

Current Florida state license as a Physical Therapist.

CPR certification.

Experience in a skilled nursing or subacute rehab setting preferred.

Strong interpersonal, communication, and clinical skills.
